Do you know your status?

Of the 1.2 million people in the United States living with HIV, it’s estimated that 13% are unaware of their status. We offer free HIV and Hep C testing. Testing is available by appointment at our Downtown Fredericksburg location - 415 Elm Street, the Culpeper office, and Mosaic Care Center.

The most comprehensive HIV care in the Rappahannock region

We provide a full spectrum of HIV care and support services designed to meet the unique needs of individuals living with HIV. From case management and medical services to housing assistance and emotional support, our holistic approach ensures that every aspect of our clients’ well-being is addressed. Explore the range of services we offer to help improve health outcomes and enhance quality of life.

Non-medical case management

Non-Medical Case Management focuses on supporting the broader social and practical needs of individuals living with HIV. Our trained staff assist clients in navigating essential support services such as housing, transportation, food access, and insurance enrollment. We work to reduce non-medical obstacles that can interfere with a person’s ability to stay in care or adhere to their treatment plan. By providing compassionate guidance and resource coordination, our team empowers clients to achieve greater stability and self-sufficiency.

Status neutral service navigation

Service Navigation provides guidance and support to all community members, regardless of their HIV status, helping individuals access the healthcare and resources they need without stigma or judgment. Individuals can connect with a Service Navigation specialist to determine eligible resources.

HIV/STI testing

Our team has been providing services and resources for people at risk of or living with HIV for over 30 years. Not only do we offer more accessible medical care through Mosaic Care Center, but clients also get connected to a caring and dedicated Case Manager who helps to facilitate connection to a variety of additional eligible services.

Housing and utility assistance

Our care team follows the 'Housing First' model, aiming to address stable housing as a primary intervention on the path to better health for our clients living with HIV. When engaged in housing programs, individuals living with HIV are more likely to gain access to and remain in treatment for their HIV, reducing the likelihood of transmission. Through our Housing programs, FAHASS offers support in securing stable, affordable housing as well as financial support for rent or utilities.

Transportation

We provide transportation to ensure that individuals living with HIV have reliable access to healthcare services and are more likely to reach their treatment goals. Clients work directly with their Case Manager to access rides for a variety of medical services.

Medical case management

Our Medical Case Management services are designed to help individuals living with HIV access and maintain comprehensive medical care. Through personalized support, our medical case managers work with clients to develop and follow care plans that address their unique health needs. This includes coordinating HIV-related medical services, assisting with appointment scheduling, ensuring adherence to treatment, and providing referrals to specialty care. By addressing barriers to care and promoting consistent engagement in treatment, we aim to support better health outcomes and improve quality of life.
